A Definitive Guide to Ankle Foot Orthoses (AFOs), bracing and custom footwear

Ensuring optimal foot and ankle health is essential for maintaining mobility and overall well-being. At Entire Podiatry, we provide tailored solutions to meet diverse patient needs, including Ankle-Foot Orthoses (AFOs), UCBLs, SMOs, various bracing options, footwear modifications, and custom footwear.

This comprehensive guide explores these devices, their applications, and key considerations when selecting the appropriate support.

Ankle-Foot Orthoses (AFOs): Enhancing Stability and Function

Conditions Treated with AFOs

AFOs are braces designed to support the foot and ankle, helping manage a wide range of conditions. Choosing between an off-the-shelf carbon fibre AFO, a Richie Brace, or a custom-made 3D-printed AFO depends on the patient’s unique needs. Common conditions treated with AFOs include:

  • Foot Drop – Difficulty lifting the front part of the foot, often due to neurological disorders.
  • Cerebral Palsy – A group of disorders affecting movement and muscle tone.
  • Stroke – AFOs help with foot drop and provide ankle support during rehabilitation.
  • Multiple Sclerosis – Assists with foot drop and enhances mobility.
  • Posterior Tibial Tendon Dysfunction (PTTD) – Affects the tendon supporting the arch, leading to flatfoot.
  • Chronic Ankle Instability – AFOs and Richie Braces help prevent further injury in people with chronic ankle pain and instability.
  • Muscular Dystrophy – AFOs assist with mobility and help maintain muscle function.
  • Spinal Cord Injuries – Provides essential lower limb support, improving mobility.
  • Arthritis – Reduces ankle pain and enhances joint stability.

Types of AFOs We Provide

At Entire Podiatry, we offer both custom-made and off-the-shelf AFOs to suit different medical needs and patient preferences.

1. Off-the-Shelf AFOs

These pre-manufactured orthoses are ready for immediate use and can be adjusted for a better fit. They provide a cost-effective and convenient solution for many conditions.

2. Custom-Made AFOs

For patients requiring a more personalized solution, we create custom AFOs using advanced 3D scanning technology. These are made from PA11 (Polyamide-Nylon), a strong and durable material.
• Maximum height limit: 38 cm due to 3D printer constraints.
• Weight limit: Not recommended for individuals over 120 kg.

For patients exceeding these limits, polypropylene serves as a reliable alternative, offering robust support while maintaining customizability.

3. Richie Braces

We also supply both off-the-shelf and custom-made Richie Braces. These combine foot orthosis with ankle support, making them versatile for treating conditions such as chronic ankle instability and posterior tibial tendon dysfunction (PTTD).

Richie Brace: A Versatile AFO Solution

The Richie Brace is a custom ankle-foot orthosis designed to treat complex foot and ankle conditions.

Benefits:
  • Improved Function – Restores natural foot and ankle movement, enhancing mobility and reducing pain.
  • Versatility – Compatible with various footwear types.
  • Custom Fit – Designed to match individual foot and ankle contours.
Common Conditions Treated:
  • Foot Drop – Helps lift the foot during walking.
  • PTTD – Supports the arch and reduces tendon strain.
  • Chronic Ankle Instability – Prevents recurrent sprains.
Considerations:
  • Customisation Time – Requires a fitting process, leading to a waiting period before use.
  • Higher Cost – Reflects the level of customisation and materials used.
  • Footwear Requirements – Due to the brace’s support, shoes must be accommodating.
UCBLs and SMOs: Specialized Orthotic Solutions
  • UCBLs (University of California Biomechanics Laboratory Orthotics): Rigid, full-contact foot orthotics designed to control severe flat feet and excessive pronation.
  • SMOs (Supramalleolar Orthoses): Braces that extend just above the ankle, often prescribed for children with low muscle tone or foot instability. They allow more movement than AFOs while still providing necessary control.

Material Comparison for AFOs

The materials used in AFOs impact their performance, durability, and comfort. Below is a comparison of PA11 (used in our custom AFOs), polypropylene (used in Richie Braces and off-the-shelf models), and carbon fibre (used in off-the-shelf AFOs).

Material

Used In

Properties Advantages Disadvantages Common Conditions Treated

PA 11 (Polyaminde-Nylon)

3D-Printed

  • Lightweight
  • Flexible
  • High impact resistance
  • Good fatigue resistance
  • Highly Biocompatible
  • Customisable through 3D printing
  • Good balance between strength and flexibility
  • Durable in various conditions
  • Limited size due to 3D printer constraints
  • Cerebral Palsy – Particularly for moderate to severe cases needing custom support
  • Spinal cord injuries
  • Muscular dystrophy
  • Posterior Tibial Tendon Dysfunction
  • Arthritis

Polypropylene (Plastic)

Richie Braces and Off-The-Shelf

  • Semi-rigid
  • Flexible
  • High durability
  • Good fatigue resistance
  • Easily moulded
  • Slightly lower biocompatibility compared to PA 11
  • Customisable during the moulding process
  • Durable and resilient
  • Less breathable compared to other materials
  • May be heavier than the composite materials
  • Posterior Tibial Tendon Dysfunction
  • Cerebral Palsy -For milder cases needing less customisation
  • Chronic Ankle Instability
  • Arthritis

Carbon Fibre

  • Extremely strong
  • Extremely stiff
  • Lightweight
  • High fatigue resistance
  • High energy return
  • Lower biocompatibility compared to PA11 & polypropylene.
  • Excellent strength-to-weight ratio
  • Provides high support with minimal bulk
  • Highly durable
  • Less flexible
  • Can be too rigid for some patients
  • Can sometime cause irritation or allergic reactions to sensitive individuals but usually encased in a protective layer
  • Stroke
  • Multiple sclerosis
  • Foot drop (various causes)
  • Muscular dystrophy
  • Chronic Ankle Instability
