If you would like to refer a patient to Entire Podiatry, please click here to download our Podiatry Referral Form. This form can be completed by doctors, health professionals, employers and/or case managers.
It is not always necessary to complete a referral form, however in some cases a doctor’s referral is required, such as if treatment is being covered through the Department of Veterans’ Affairs (DVA), or a Medicare Care Plan.
Completing a referral assists us by providing a basic summary of the reason for the referral, relevant results obtained to date, as well as any specific requests.
